The Federal Voting Assistance Program has created this postcard to enable Local Election Officials to communicate with voters. This postcard will allow you to inform the voter that:

The voters Federal Post Card Application has been denied, with cause

The postcard is available here in PDF format: www.fvap.gov/leo/leocard.pdf. The file is optimized to print a double-sided copy on an Avery Form 8387 postcard for Ink Jet Printers (can be printed on a laser printer as well). Office Depot Item Number 915-895 or equivalent can also be used.
